Number 50 values the house call and expressly includes consultation and symptom-related examination — which is why numbers 1 and 5 may not be billed alongside it. 320 points give 18,65 € at the base rate, 42,90 € at 2.3× and 65,28 € at 3.5×.
GOÄ number 50 in the regulation's own words
The word “including” carries the whole system here: what is included cannot also stand alongside it, and the provision under the number says exactly that, explicitly, for numbers 1 and 5.
The regulation distinguishes the house call from the hospital visit (Visite). Numbers 45 and 46 are hospital visits; number 50 may not be billed in place of or alongside them. Conversely, the house call fee is excluded for hospital and attending physicians working within a hospital.
Where a further patient in the same household is visited in immediate temporal connection, the regulation provides number 51 for that — again including consultation and symptom-related examination.
The sentences below are quoted here exactly as they stand in the Gebührenverzeichnis (fee schedule) — as the general provisions attached to number 50. The German wording is the binding legal text; the words after the dash are a working translation for orientation only, not a substitute for the original.
- „Die Leistung nach Nummer 50 darf anstelle oder neben einer Leistung nach Nummer 45 oder 46 nicht berechnet werden.“ — number 50 may not be billed in place of or alongside number 45 or 46.

The multiplier (Steigerungssatz) for number 50
Number 50 stands in Section B, Subsection IV, and so within the usual frame: from the base multiplier up to 3.5×, with the usual range running up to 2.3×.
For numbers 45 to 62, the regulation provides separate surcharges under letters E to J and K 2, which may only be billed at the base fee rate. They must be listed on the invoice directly after the underlying service.
Nothing special applies to the multiplier itself: § 5 Abs. 2 governs the assessment, § 12 Abs. 3 the written justification above the 2.3× multiplier.
What the record must contain for number 50
A house call happens where the practice software is not. The record is therefore regularly made in two stages: a note on the road, an entry back at the practice — and between these two steps sits the point where § 630f Abs. 1 BGB requires immediate temporal connection.
Because consultation and symptom-related examination are part of the service, they also belong in the record. An entry that says only “house call” captures neither the content of the consultation nor the finding.
Anyone who, following a house call, sees a further patient in the same household has documented two treatments, not one. The regulation separates them into two numbers; the record separates them into two cases.
Frequently asked questions
May number 1 be billed alongside number 50?
No. The wording already includes the consultation, and the general provision states expressly that numbers 1, 5, 48 and/or 52 may not be billed alongside number 50.
What applies for a second patient in the same household?
